Improving Outcomes for Homeless People Discharged from Hospitals

People who are unsheltered commonly get discharged back onto the streets after a hospital visit; and while they may be "stable", their living situations are not. After the recent death of a Salem woman in this situation, State Sen. Deb Patterson introduced SB 1076 to give discharged patients w/o a permanent address a "warm hand-off" to a social service agency as one of several new discharge procedures provided in the bill.
